Hi, What is the source of this chant? I've been trying to sort out the variants on the CPDL text page, but expert help is much appreciated!

Does anyone have a this chant written out in modern notation, preferably with organ accompaniment? I'm asking for a friend who doesn't read square note. NOH has accompaniment for the other tune, but this one by Pothier is the one she needs.
