Hymn for the Year of Faith

Supposedly there's an official hymn for the Year of Faith. Does anyone know where I can get the music in English or Spanish?.

The title is Credo, Domine.

I used "Faith of Our Fathers," with original text to keep from using that dog of an official hymn. It is really bad.
