Musical Shape of the Liturgy ship to Canada?

I would like to own a copy of Mahrt, The Musical Shape of the Liturgy. The CMAA site proposes Amazon as the source. Amazon.com said that it cannot be shipped to my (Canadian) address. Amazon.ca only offers the "Kindle" download -- but I would like an actual book.
Have I missed a downbeat somewhere? Is this book available in Canada? -
