There are a number of online resources suggesting the praying of the Litany of the Saints for those who are near death, with the usual "pray for us" responses replaced with "pray for him/her" (i.e., ora pro eo/ea). The MTF Handbook of Prayers, for one, recommends that the litany be "said or sung". I remember this was sung for Pope John Paul II, in Latin, on the day before his death.
Not sure whether this is an old practice or modern, but wondering if anyone has music for it handy, in Latin and/or English?
