Trying to locate music for this Polish Hymn

If anyone knows where I can find the sheet music for this, I would be grateful... or else anyone with a good ear that has time! I just want the melody line (soprano).

All I can find out about it is that its by a certain passionist nun named Sr. Ancilla. A polish sister wanted to try and do an English rendition.
