Does anyone recognise this 'O Salutaris' setting?

I'm trying to find this version of O Salutaris with the words beneath to print.

It says 'from Beethoven' in the screen shot, but nothing similar is appearing in Google when I search for 'O Salutaris Beethoven'.

I certainly recognise that from my childhood, 75 years ago, but I do not know it's provenance.

The tune is known as St LUKE, or as COCKFIELD GREEN. Composer seems to be unknown, early 19th century.

It's certainly a lovely version. I sung it many years ago - it was the go-to version for benediction in my particular church at the time. But I never knew who it was by or where to find it after I left.
